As part of your medical records we hold information on a number of matters relating to a persons health including vaccinations, allergies, medications both past and current, and any significant medical matters either current or past. If you would like to view your medical records, please make an appointment with the Practice Manager by ringing 01677 422616. There may be small charge for this and any copies of records required and the Practice Manager will discuss this with you before your appointment.
If you wish to access information such as past vaccinations, please contact reception on 01677 422616 to discuss this further. It would be appreciated of such requests for non-urgent matters could be made between 10am and 1pm.
